What is a CBRF?

The State defines a CBRF as a place where five or more adults reside:

  • who are not related to the operator or administrator
  • who do not require care above intermediate level nursing care
  • who receive care, treatment or services that are above the level of room and board, but includes no more than three hours of nursing care per week per resident.
CBRF's range in size from 5 to 257 beds. Birch Creek is certified for 84 beds.

Each CBRF has specialty programs. Birch Creek is licensed for Advanced Age and Terminally Ill.

All CBRF's must adhere to Chapters HFS 12 and HFS 13, Wisconsin Administrative Code ~ Caregiver Background Checks and Allegation Reporting.

All CBRF's are licensed by the State of Wisconsin Department of Health and Family Services. State licensing specialists inspect CBRF's via unannounced surveys.

The goal of a CBRF is to assist individuals in achieving the highest level of independence of which they are capable. CBRF's provide assisted living services above normal room and board such as assistance with medications, personal care and supervision. Assisted living services may be provided up to 24 hours per day, seven days a week. The philosophy of a CBRF is to provide a home-like environment and to keep each resident as dignified and independent as possible, while maintaining a feeling of safety and security.

Are there different types of CBRF's?

There are three sizes of CBRF's based on the number of residents; small (5-8), medium (9-20) and large (21 or more). They also differ by classification, which is determined by the resident's ability to move about (ambulation), to follow directions (mental capacity) and to act for self preservation in emergency conditions.

CBRF's must provide the following services:
  • Health monitoring
  • Assistance with medications
  • Help with daily personal activities such as bathing, dressing, toileting, mobility, nutrition and medications
  • Leisure activities in both the facility and community
  • Information and referral services
CBRF's must also provide:
  • A program statement, which is a description of the program goals of the facility consistent with the needs of the residents to be served, and a full description of the care and services to be provided. This allows prospective residents to become familiar with the services a CBRF offers
  • An individualized service plan (ISP) for each resident
  • A list of resident's rights
  • A complaint procedure outline
